- Healthcare Coordination Tool Kit - The Healthcare Coordination Toolkit is designed to assist individuals, guardians and teams in the proper coordination of healthcare for adults receiving DD Waiver funded supports. The forms included in this toolkit are guides of the information that should be tracked, but the forms are not required.
The intent of this toolkit in the following six sections below is to assure that individuals receive timely medical follow-up, testing, and monitoring for adequate healthcare delivery.
