Model Employment Project

Reported information on the model includes:

• number of days and hours worked each month,
  
• description of primary work assignments for a given quarter,
  
• description of informal and formal activities that promote inclusion with faculty and customers of the facility,
  
• status report of formal/informal job accomplishments, including notable enhancements of job skills, increased level of independence and/or quality of work,
  
• description of paid and natural supports provided to enhance the quality and independence of work and full integration into the formal and informal operations of the center,
  
• description of training provided to staff of the center, building and/or other self-advocates, guardians/advocates, case management, therapist and/or community provider staff, through observation and/or description of this project, and
  
• any other anecdotal information the CONTRACTOR may wish to share about the self-advocate employees and/or project.
