New Mexico birth and death certificates are restricted access records.

 

New Mexico Vital Records are restricted access records and are only issued to immediate family 

members or individuals who demonstrate tangible legal interest

 

 

Information on how to obtain a New Mexico birth certificate in person

Counter Service for requesting birth certificates through the Santa Fe State Office is available Monday 

through Friday from 8:30 a.m. until 4:00 p.m. Counter hours are subject to change during Holidays or other 

infrequent closings. The Santa Fe State Office is located at 1105 South Saint Francis Drive. The office is 

located on Saint Francis Drive between Cordova Road and Alta Vista Street. The telephone number is

 (505) 827-0121.

 

Counter Service for requesting birth certificates through the Albuquerque Stanford Public Health Office is 

available Monday through Friday from 9:00 a.m. until 4:00 p.m. The Albuquerque Office is housed at the 

Stanford Public Health Office at 1111 Stanford NE. The telephone number is (505) 841-4185.

 

The person making the request must complete an application available at the office and present a current 

government issued picture identification. The search fee for a birth certificate is $10.00 per certificate. 

 

 Information on how to obtain a New Mexico birth certificate by mail

To obtain a birth certificate by mail:

1. You may print out and mail (do not email) the birth search application form 

            Birth Search Application Form   (Note: Do Not Email)  PDF format

OR

2.  You may send a letter with the following information:

The birth name of the person on the certificate.

The date of birth.

The city of birth. Include county if known.

The mother's full maiden name.

The father’s full name.

Requestor's relationship to the person on the birth certificate.

The purpose for which you are requesting the birth certificate.

Your name and signature.

Your mailing address.

If you receive your mail at a Post Office Box, please include a physical (street) 

address for our internal files.

The fee for a birth certificate is $10 per certified copy. Please make your certified check or money 

order payable to "New Mexico Vital Records".

 

Do not send cash.

Please mail your request along with the correct fee to:

Please allow 4 weeks for processing.

 

Adoption

To amend a New Mexico birth certificate due to an adoption, please submit a certified copy of the 

"Certificate of Adoption" form. This form must be certified by the clerk of the court and must include all 

information necessary to locate the original Certificate of Birth.

 

The Certificate of Adoption must be completed in black ink and must not contain errors, 

cross outs, or white out.

 

If the child was born in a State other than New Mexico, Certificate of Adoption forms will be forwarded to 

that State of birth. No further action will be taken.

If the child was born out of the country and adopted by legal residents of the state of New Mexico, the 

Certificate of Adoption form must be accompanied with an order issued by the court ordering New Mexico 

Vital Records to establish a certificate of birth for that child.

 

Acknowledgment of Paternity

If the mother was unmarried at the time of the child’s birth, the biological father’s name may be 

added to the birth certificate by filing an "Acknowledgement of Paternity" form. The 

Acknowledgement of Paternity form must be signed by the natural parents in the presence of a
