Community-Based Adult Habilitation Best Prectices, Provider Success
Tobosa Developmental Services of Roswell was selected as the provider that best demonstrated the support of an individual's progress in the community. As one example, Tobosa supported Domingo Romero with community-based Adult Habilitation and Community Access Services and helped him become a successful community member. Because of the success of their community-based Adult Habilitation program, Tobosa staff presented their ideas on community-based services and supports at the 2006 Community Solutions Summit. Tobosa staff described how they successfully changed from segregated adult day habilitation services for people with developmental disabilities to community inclusive support services using the Person Centered Approach. Best practices were implemented through individualized Interdisciplinary Team planning and staff practices using community and generic supports whenever possible. Tobosa has a strong commitment to community inclusion for peopel with developmental disabilities. For example, in 2003 Tobosa secured a grant from the Governors Commission on Disability and Quality of Life for $8,000 to support the opening of a People First organization in Roswell for Chavez County. With Tobosa's help, People First grew in numbers and more self-advocates participated in the organization. In addition, a speaker's bureau was developed and those who became speakers helped to provide a strong presence all over the state of New Mexico.
