2010 DDSD Statewide Director’s Community Inclusion Awards
The Developmental Disabilities Supports Division (DDSD) honored the following individuals, community organizations and business for demonstrating excellence in Community Inclusion. Awards were given in the categories of business ownership, outstanding employee of a business owner, community based adult habilitation, individual community membership, team champions supporting individual community membership, full community inclusion, case manager employment champion, employee of the year and employer of the year.

2010 DDSD STATEWIDE DIRECTORS COMMUNITY INCLUSION AWARDS

Ms. Dawn Boelkins is the 2010 individual award winner who takes part in services from the award winning provider for Community Based Adult Habilitation, Cornucopia. Dawn has experienced great personal growth as a result of being seen as another equal member of the community with a strong voice.

Dawn benefited from Cornucopia’s one-on-one community opportunities, "Make A Difference" strategies, an emphasis on developing natural supports, and Community Access Services.

Through the "Make A Difference" process implemented at Cornucopia with the support of Metro Community Inclusion Meaningful Day Coordinator, Lori Drury, she started getting massage therapy and made friends with the massage therapist. She also began having regular hair and nail appointments.

Dawn now goes to church twice a week where she is part of a young women’s club. Friends she has made there invite her for overnights.

Cornucopia enlisted volunteers as natural supports and works with therapists to deliver therapies in the community.
