2010 DDSD Statewide Director’s Community Inclusion Awards
The Developmental Disabilities Supports Division (DDSD) honored the following individuals, community organizations and business for demonstrating excellence in Community Inclusion. Awards were given in the categories of business ownership, outstanding employee of a business owner, community based adult habilitation, individual community membership, team champions supporting individual community membership, full community inclusion, case manager employment champion, employee of the year and employer of the year.

2010 DDSD STATEWIDE DIRECTORS COMMUNITY INCLUSION AWARDS

Mr. David (Teddy) Abeyta is the 2010 Individual Community Member Award Winner.

Teddy’s art work, shown on this slide, is displayed in many places in the community. He works on his drawings and paintings on a daily basis using markers and painting with acrylics. Teddy is working to display his art at Spanish Market. He is already receiving recognition from the Santa Fe Art community for his work.

Teddy is employed at Vision Case Management and provider Santa Lucia supported by natural supports doing maintenance and clerical duties. He is meticulous with detail, has a strong work ethic and takes pride in a job well done.

Teddy also volunteers at Homewise, a program assisting first time home buyers, doing filing and copy making.

He goes to concerts in the plaza, Isotopes games, parades, family posting events, fiestas.

Teddy also works out. He trained for and completed a 5 k race.

He attends the community college. He also spends a lot of time with family going to picnics, parties and concerts.

With natural supports, Teddy studied for almost a year and received his confirmation at Santa Maria de la Paz May 2009. He sings the recessional hymn at the end of mass.

He leads his Interdisciplinary Team meetings and his Individual Service Plan (ISP) reflects what he wants in his life.

Teddy has many friends, including the mayor of Santa Fe.
