2008 Statewide Directors Meeting Break-Out Session Outcomes for Jackson Disengagement Strategies

The Developmental Disabilities Supports Division (DDSD) provided a series of information sharing and strategy sessions to improve critical Jackson Lawsuit disengagement issues.  The outcomes were offered as possible solutions for disengagement.  The outcomes are listed under each of the following Break-Out Session Topics:

blueblip Aspiration Risk Management facilitated by Betsey Finley

blueblip Healthcare Coordination facilitated by Jennifer Thorne-Lehman and Travis Goldman

  • Support individuals, families and direct care staff to perform the function of Healthcare Coordination and share information in a meaningful way with the team.
  • Use the Healthcare Coordinator as an information center, not as guiding medical procedures or care.
  • Develop a "Resource Kit" to support the individuals to have quality healthcare and healthcare coordination.
  • Use the Health Risk Screening Tool (HRST) to track medical information.
  • Assure that the HRST has the information input every time someone has a medical event (pneumonia, emergency room visit, diagnoses, doctors' appointments, medication change, etc.).
  • Use the HRST to guide further medical needs.
