Medically Fragile Condition:
Medically fragile is defined as a chronic physical condition that results in a prolonged dependency on medical care that requires daily skilled nursing intervention. A medically fragile condition is characterized by: a life threatening condition requiring frequent medical supervision and/or consultation of a physician, individuals require administration of specialized treatments, and individuals are dependent on medical technology such as ventilators, dialysis machines, enteral or parenteral nutrition support and continuous oxygen.
