Case Management services are person-centered and intended to support an individual in pursuing his or her desired outcomes by facilitating access to supports and services.  Case Management is a set of interrelated participation of the individual and/or his or her designated representative (e.g., guardian).  Case Managers:

  • Complete assessment process
  • Facilitate service planning process
  • Monitor the implementation of services
