Developmental Disabilities Services:
The Developmental Disabilities Waiver Program helps eligible New Mexicans with developmental disabilities maintain and develop skills needed to live a quality life in their local communities. Services can be received from only one Medicaid Waiver Program at a time, even if the individual qualifies for more than one Waiver. There are several waiver programs in addition to the Developmental Disabilities Waiver. State General Funded services are available for those who do not meet medical and financial eligibility requirements for the Developmental Disabilities Waiver Program. Services are available include respite (birth to adult), residential services and vocational habilitation services. Services Available Through the DD Waiver Program:
Services include Case Management, Day Services, Living Supports, and Therapies (Physical, Occupational Speech/Language and Behavioral). Other services and supports include Independent Living Transition Service, Community Integrated Employment Services, Customized Community Supports, Customized In-Home Supports, Adult Nursing, Nutritional Counseling, Personal Care, Crisis Support, Supplemental Dental Care, Respite, Environmental Accessibility Adaptations, and Non-Medical Transportation. Evaluation services include Specialty Seating and Wheelchair Evaluation Clinic, a Supports and Assessment for Feeding and Eating (SAFE)Program and Augmentative and Alternative Communication (AAC) evaluation and Aspiration Risk Evaluation services. How to Apply:
Developmental Disability Criteria is defined as a combined condition of Mental Retardation or Related Condition plus Functional Limitations in 3 Life AreasRegistration for the DD Waiver can be made in person or by phone at the Developmental Disabilities Supports Division (DDSD) Regional Offices or in person at the local Income Support Division (ISD) Office.

If registration is made by phone at the DDSD Regional Office, the Eligibility Worker will complete the Waiver Services Registration Form (MAD 325), date stamp and initial when the form was completed.  DDSD will mail an Application Packet to the registrant or the registrant's representative that includes a copy of the date stamped MAD 325.

If the registrant comes into the DDSD Regional Office to complete the MAD 325, a copy of the date stamped form will be provided to the registrant or registrant's representative.  To see list of addresses and phone numbers of DDSD Regional Offices click here.

Registration can also be made in person at the local Income Support Division Office.  The registrant will be given a date stamped receipt.  ISD will mail the registration form to the local DDSD Regional Office for processing.  To find your local Income Support Division office, please go to www.hsd.state.nm.us/isd.
