State General Funded Services: Referrals, Signing Up for Services and Availability of Services in your Location
Making referrals, signing up and service availability is coordinated the five regional offices. See Regional Office Locations

Making Referrals
To refer an individual for State General Funded services, refer the individual and/or their family to the Eligibility Management Unit at 1-800-283-5548.

Signing Up for Services
Once an individual is signed up through the registration process through the Eligibility Management Unit, the Eligibility Worker will give the list of eligible individuals to the Providers of that Region (see regional map for more information). This is done on a first come, first serve basis.

Availability of Services
Eligibility for services does not guarantee placement in services. Services are limited based on State Legislative Appropriation and availability of providers for service in your area.
