Supported Employment Transition Program

Eligibility Determinations
Individuals wanting vocational services would be recommended by the current Individual Employment Plan (IEP) or Interdisciplinary Team (IDT) for this service. Criteria for recommendation should be based on the following factors: 1. Individuals are awaiting DD Waiver services, 2. Individuals desire to obtain employment for a minimum of 10 hours per week, 3. There is an opportunity for employment where the recipient earns minimum wage or above, and 4. Employment can be obtained in a community-integrated work setting.
