Supported Employment Transition Program

Transitioning
The Interdisciplinary Team (IDT) writes a transition plan for each individual to transition from receiving only high school services to Student Employment Services. The transition plan may be included in the initial Student Employment Services plan or may be in the form of IDT minutes but must be developed by the current IDT. The transition plan must include the date the Student Employment Services will begin, identified needs, supports the individual is currently receiving and must detail how the individual will meet his/her vocational vision. The transition plan must be written and agreed upon by all team members before the transition of services takes place.
